For provider organizations

Document the days. Visit on the days that matter.

careib.io captures what a home health visit would have observed — every day, automatically. Your nurses still visit. They just visit the patients who actually need them today, with documentation already in hand. Same outcomes. Fewer hours. Less burnout.

Built for the organizations moving care home

🏠

Home health agencies

Document daily for patients on Medicare home-health benefit. Reduce per-patient visit frequency without losing documentation density. Make your highest-acuity visits count.

🕊️

Hospice and palliative care

Capture symptom progression, conversation memory, and medication compliance for patients in the home setting. Family caregivers feel supported, not abandoned.

🧠

Memory care providers

Higher-acuity dementia care delivered at home. DOT methodology gives families confidence. The conversation memory layer means a patient with declining recall isn't losing their own life story.

🏥

SNF discharge teams

Move high-risk discharges out of the SNF earlier with continuous documentation in place. Lower readmission risk; happier patients and families; bed available for the next admit.

👨‍⚕️

Concierge MD practices

Premium home-based primary care delivered with hospital-grade documentation. Differentiator for the practice; comfort for the patient.

🏢

Self-insured employers

Care for aging parents of high-value employees, delivered at home, paid by the employer benefit. A retention play disguised as a healthcare benefit.

Late-stage and advanced impairment

For patients who can't reliably wear a device.

Wearable compliance falls off a cliff with advanced cognitive impairment, in hospice, and for any patient who spends most of their day in a single room. careib.io supports both architectures so the technology fits the patient — not the other way around.

  • Wearable Plaud — the right choice for active early- and mid-stage patients still moving through their day.
  • Patient Room Hub — a stationary Plaud Desktop running on a Mac Mini in the patient's primary room, paired with a directional microphone aimed at typical conversation zones. Captures patient, family visits, nurse visits, phone calls, and the TV — without depending on the patient putting anything on. The same Mac Mini hosts the Insta360 DOT capture for the medication station, so a single low-profile box runs both audio and video pipelines.

Two product variants. Same backend. Same clinical report at 7 PM.

One-time hardware cost, paid by the agency, deployed by your local installer. The Plaud Desktop subscription, careib.io platform tier, and ongoing operating costs sit alongside.

The economics

The math on a single 50-patient agency.

Without careib.io

  • 50 patients × 3 nursing visits/week = 150 visits/week
  • 150 visits × $150/visit = $22,500/week in cost
  • ~6 FTE nurses to cover
  • Documentation in arrears, often nights and weekends
  • One missed dose between visits = no record until next week

With careib.io

  • 50 patients × 1.5 nursing visits/week = 75 visits/week
  • 75 visits × $150 + 50 × careib subscription
  • ~3 FTE nurses + careib platform
  • Documentation in real time, in the patient record
  • Every dose documented, audit-ready

Net savings vary by tier and patient mix. The illustration assumes the DOT Clinical tier at $499/patient/month. Talk to us about the volume pricing that fits your size — single-billing arrangements are standard for agencies above 25 patients.
